mindspan uses telehealth to provide you with a lab order for the Optimism Biology Panel. This consent explains how that review works, its limits, and your rights. Please read it before you check the consent box on the intake form.
What telehealth means at mindspan
A licensed clinician (physician, nurse practitioner, or other qualified provider) reviews the health intake form you submit through our website and, if appropriate, issues a lab order so you can have your blood drawn at a Quest Diagnostics location.
This review is performed asynchronously — the clinician reads your intake at a convenient time and does not require a real-time video or phone visit unless they determine one is medically necessary. Florida law (Fla. Stat. §456.47) permits the use of synchronous or asynchronous ("store-and-forward") telehealth to provide health care services.
Limitations of telehealth
By consenting to telehealth services, you acknowledge that:
- The clinician will rely on the information you provide in your intake. Inaccurate or incomplete information may lead to an incorrect order or interpretation.
- There is no in-person physical examination unless the clinician requests one.
- Telehealth is not appropriate for emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
- If your results show values outside critical safety ranges, the reviewing clinician may contact you to recommend follow-up with your primary care provider.
- Technology failures, transmission errors, or unauthorized access — although safeguarded — remain possible risks of any internet-based service.
Confidentiality
Information shared during your telehealth encounter is protected the same way as in-person care. Your data is encrypted in transit and at rest, and we maintain a HIPAA Notice of Privacy Practices describing exactly how your protected health information is used and disclosed — see our Privacy Policy & Notice of Privacy Practices.
Your rights
- Right to refuse: You may withhold or withdraw your consent to telehealth at any time without affecting your right to future care.
- Right to alternatives: If you prefer an in-person evaluation, you may decline mindspan's service and seek a comparable lab order from your own provider.
- Right to a copy: You may request a copy of this consent and your intake submission at any time by emailing hello@joinmindspan.com.
- Right to ask questions: Reply to any mindspan email and we will route your question to the reviewing clinician where appropriate.
What you are consenting to
By checking the consent box on the mindspan intake form, you confirm that:
- You have read and understood this Telehealth Informed Consent;
- You have read and understood mindspan's HIPAA Notice of Privacy Practices;
- You are at least 18 years old;
- You consent to receive a lab order for the Optimism Biology Panel through asynchronous telehealth;
- You understand mindspan is a wellness and educational panel, not a diagnostic test, and your report does not constitute medical advice, diagnosis, or treatment.
